Immanuel Participant Center Aide - Full Time Days - Immanuel Pathways Central Iowa in Windsor Heights, Iowa
Responsible for providing and assisting participants of Immanuel Pathways program with activities of daily living, restorative and supportive care. Assists with the organization and implementation of activity programs. Monitors health status and provides a safe environment for participants. Aides function under the direct supervision of the Participant Center Manager, working with indirect supervision from the department they are supporting. Departments include: Occupational Therapy, Physical Therapy, Clinic, Dietary, Recreational Therapy, and Transportation. Function as an Interdisciplinary Team member. Supports and lives out Immanuel’s Mission and CHRIST Promises.
Requirements of Job:
Be legally authorized (certified) to practice the job’s functions and actions in Iowa and Nebraska.
Only act within the scope of his/her authority to practice in Iowa and Nebraska.
Agree to abide by the philosophy, practices, and protocols of the PACE organization
Job specific competencies for the Participant Center Aide will be met prior to assuming participant care.
Incumbent may be assigned to perform job duties or cover for a counterpart at more than one Immanuel Pathways site in Iowa and Nebraska on a fill-in basis for a short duration.
Key Responsibilities and Duties of the Job
85% Participant Care and Assistance
Assists and monitors participants with activities of daily living (ADL), including but not limited to:
Bathing, dressing, transfers, hair care, grooming, nail care, shaving, oral hygiene and toileting
Laundering participant clothing at the center
Noting changes in participant’s physical or emotional status and communicating information to appropriate department/discipline
Complete daily charting
Assists with participants that have memory support needs
Assists in the memory specific room
Assists participants with Clinical needs including but not limited to:
Lymphedema treatments, vitals, specimen collection, other tasks as directed and competencies completed.
Collaborates with participant and clinic regarding triage needs.
Escorts participants to clinic for clinic appointments when directed.
Follows Immanuel Pathways documentation standards for participant care.
Assists participants with Transportation needs, including but not limited to:
Accompanying participants to and from home with the drivers
Accompanying participant to medical and other appointments
Helping the driver load and unload participants from vehicles
Assists with Recreational Activities, including but not limited to:
Creating and participating in group and one-on-one activities in accordance with daily activity calendar and/or requested by Recreational Therapist
Preparing for, facilitating and cleaning up participant activities
Assists with participant Dietary needs, including but not limited to:
Supporting the Registered Dietitian and Dietary Manager with setting up, serving and cleaning for snacks and noon meal
Assisting participants with specific dietary needs (i.e. thicken liquids, feeding, etc.)
Dietary Documentation (i.e. weights, meals etc.)
Assists with participant Therapy needs, including but not limited to:
Restorative exercises on behalf of Physical Therapy
Participant specific Occupational Therapy interventions in the center
Functions as a IDT member (as assigned) and supports participant care plans:
Participating in the activities of the Interdisciplinary Team
Carries out the plan of care as directed
Maintain a, safe, welcoming and clean environment for participants
Daily communication with Participant Center Manager
Maintain clean day rooms and personal care areas
Treating participants with compassion, dignity and respect

Using proper lifting and push/pull techniques, use gait belt assistance and participant transfers
Follows proper equipment use and cleaning
Maintain Nursing Assistant certification
- Perform other duties as required or requested
High school diploma or equivalent is required.
Active Certified Nursing Assistant Certificate required.
- One (1) year of experience working with the frail or elderly population required, or completion of job specific training related to working with the elderly population must be completed within the first six months of hire.
Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
Basic Life Support (BLS) Skills i.e. Health Care Level Cardiopulmonary resuscitation (CPR); Automated External Defibrillator (AED); First Aide (FA).
KSA- Knowledge Skills and Abilities-
Experience in identifying and recommending problem resolution.
Experience in establishing and maintaining effective working relationships with participants, co-workers, and the public.
Ability to communicate clearly and effectively with co-workers and Participant Center Manager.

Proven experience and basic computer proficiency (internet, email, Microsoft).
