UnityPoint Health RN Lead-PACE (Fulltime Days) in Sioux City, Iowa

Under the supervision of the PACE Clinical Manager, coordinates and supervises delivery of personal care services for Siouxland PACE participants. Provides direct supervision of personal care attendants and direct care contract staff; monitors in-home care & service delivery to ensure conformance with established Siouxland PACE quality assurance and operational policies, procedures and standards. Adheres to the principles of Service Care Standards and the philosophy, mission and values of the Siouxland PACE program in all aspects of job performance.

Key Accountability

  • Maintains a working knowledge of applicable Federal, State and local laws and regulations, the Compliance Accountability Program, Quality Assessment and Performance Improvement and Code of Ethics, as well as other policies and procedures, in order to assure adherence in a manner that reflects honest, ethical and professional behavior.

  • Documents all participant care related activities accurately, legibly, and in compliance with Siouxland PACE policies.

  • Is knowledgeable of and complies with Siouxland PACE Safety and Infection Control Policies and Procedures.

  • Adheres to Siouxland PACE Participants Rights & Responsibility Policy at all times.

  • Protects privacy and maintains confidentiality of sensitive participant, employee and agency information.

  • Works sensitively and effectively with individuals of diverse ethnic and cultural backgrounds.

  • Performs other duties as assigned.

  • Functions as a collaborative member of the interdisciplinary team; promotes an environment of teamwork across all PACE settings.

  • Conducts history and functional nursing assessments of potential participants, including the Home Safety Evaluation. Arranges transfer of durable medical equipment to contracted suppliers as appropriate for newly enrolled participants.

  • Develops initial proposed plan of care in collaboration with the Interdisciplinary team (IDT) for new Participants, Informs team of changes in health or functional status of participants in relation to home care needs.

  • Performs annual, semi-annual and as needed in-home and facility assessments of participants, and as indicated and updates home care plans, as indicated.

  • Conducts PCA required and as needed in-home supervisory visits to assure consistency and quality of care is delivered by personal care attendants to meet established standards.

  • Fosters teambuilding and professional relationships among staff and other healthcare providers.

  • Provides oversite and assists with scheduling PCA services; ensures that participants are allocated time/services per plans of care.

  • Responsible for coordination and oversight of contracted PCA services.

  • Responsible for direct oversight of PCA staff performance and implementation of awareness discuss and/or verbal coaching.

  • Collaborates with interdisciplinary team and contracted providers regarding complex home care situations. Serves as liaison with participants, families and teams regarding complex home care needs and plan of care.

  • Develops appropriate content and conducts PCA meetings.

  • Practices and reflects knowledge of HIPAA, TJC, DNV, OSHA and other federal/state regulatory agencies guiding healthcare.

  • Completes all annual education and competency requirements within the calendar year.

  • Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff. Takes appropriate action on concerns reported by department staff related to compliance.

Education:

  • Minimum Licensed as LPN or RN.

Experience:

  • Two years’ experience in medical field such as a clinic, long term care facility, or hospital.

  • Minimum of one year of experience working with the frail or elderly population

License(s)/Certification(s):

  • Maintains current CPR status or acquire within 30 days of hire.

  • Maintains current Mandatory Reporter status or acquire within 30 days of hire.

  • Valid driver’s license when driving any vehicle for work-related reasons.

  • Valid Auto insurance
