UnityPoint Health Health Information Management Clerk-PACE in Sioux City, Iowa

  • Under direct supervision, primary responsibility is to assure completeness and accuracy of patient medical records. Duties include: scanning, filing, communicating patient information updates, maintaining adequate systems for locating, filing, and retrieving medical records and tracking physician orders. Adheres to the Service Care Standards and the philosophy and mission of Siouxland PACE in all aspects of job performance.

Department Support

  • Maintains complete and accurate patient files and completes electronic scanning and filing of documents on a timely basis.

  • Consults with primary care and nursing staff, as well as other related departments in gathering medical information necessary to maintaining current, accurate and comprehensive medical records.

  • Reviews health records for completeness according to established criteria and informs direct supervisor of deficiencies.

  • Establishes clinical records for new admissions.

  • Updates patient records with required information to assure accurate daily census and clinical information.

  • Participates in chart audits designed for quality assurance and utilization review programs.

  • Obtains medical records and discharge summary for participants who have been hospitalized outside the Iowa Health System.

  • Verifies that documentation from outside providers is present in the EMR

  • Prepares copies of PACE Provider records, visit notes and assessments for coding and billing staff review.

  • Maintains an organized filing system and an orderly medical records area.

  • Processes internal authorized requests for medical records in compliance with Siouxland PACE policies and procedures.

  • Processes external requests for release of member medical records according to federal, state and Siouxland PACE guidelines, and submits requests for information as directed by the supervisor Serves as a second receptionist when needed.

Education:

High school diploma or GED.

Experience:

Prefer one year work experience in a hospital or medical record department, physician office or other health care setting. One year experience serving the elderly population

License(s)/Certification(s):

Valid Driver’s License

Valid Auto Insurance

Knowledge/Skills/Abilities:

Expert level of computer literacy. Communication skills written and verbal, planning and organizing, prioritizing, problem assessment and problem solving, information gathering and information monitoring, attention to detail and accuracy, flexibility, adaptability, customer service orientation, teamwork.
