Responsible for facilitating a direct communication link with the regional primary care physicians whose patient has been recently transferred to Mercy Medical Center via an outlying acute care facility, including data records. Responsible for meeting with patients and family members who were transferred to Mercy Medical Center to assist with any immediate needs.


  • Conducts notification calls to regional primary care physicians (PCP) of their patient’s admission and status, within 24 hours of admission.

  • Contacts regional PCP with any patient change in status or discharge including notification of expired patient.

  • Provide follow-up with patients and their families who have been transferred to Mercy, within 24 hours of admission.

  • Field regional primary care physician/clinician questions and provide immediate follow up such as providing medical record documents.

  • Maintain continuity of care from time of discharge to follow up visit with patient’s regional PCP/specialist.

  • Establish point of communication between outlying physician/clinician and Mercy.

  • Update patient demographic information in electronic medical records that was not available/collected at time of admission

  • Review Detailed Transfer Log from previous day and summarize transfer requests, accepted and non-accepted requests. Provide summary report to director.

  • Track payer mix on all accepted transfers and provide daily/weekly summary report.



  • Possession of a current or compact state license as a registered nurse issued/defined by the State of Iowa.

  • BSN preferred.

  • Minimum 2 years of critical care nursing experience required

  • Proof of completion of Mandatory Reporter – Child and/or Dependent Adult Abuse training within three (3) months of hire.

  • BLS certified by the end of the orientation period (approximately six (6) weeks).
