The Licensed Practical Nurse (LPN) is a community based nursing position that will provide services to persons served by the Assertive Community Treatment (ACT) program in their home or communiyt. The LPN will coordinate care to ensure that psychiatric care, physical health issues, and medication issues are integrated into treatment. The RN attends psychiatry appointments and ensures all needed follow-up is completed.
The incumbent in this position is a mobile clinician and expected to deliver services to individuals throughout the service territory as scheduled. This position will also support the operation of the program on a rotating on-call, evening, and weekend schedule.
Implement, maintain, and coordinate the medical treatment for ACT consumers.
Perform medical functions regarding basic health, medical assessment and education.
Provide locked security for medications and set aside a private designated area for set up of medications.
Administer medications and provide injections to consumers as directed by doctor
Check consumer medication trays and fill them as needed
Provide services at times and locations convenient to consumers.
Provide on-call, after-hour support to consumers as scheduled.
Provide face-to-face services in the consumer's home, in public places, wherever necessary.
Ensure consumers have access to and utilize the ACT after-hours on-call staff for emergencies or after-hour support.
Provide transportation to medical appointments or other agency events as necessary.
Provide assertive outreach when necessary to engage consumers.
Submit accurate and on-time chart documentation and reports.
Preferred prior experience working with persons with severe mental illness and substance use/abuse issues
Licensed Practical Nurse Licensed in the state of Iowa
The ability to make ethical clinical decisions in demanding situations
The ability to interact professionally with clients, their families and other professionals
Organizational skills; ability to complete documentation timely and accurately

The RHD Cedar Rapids ACT program serves a 60-mile radius from the Cedar Rapids office.
